Diabetes Self-Management Classes (Physician Referral Required)

Small group, multiple sessions for individuals with type 2
diabetes. Gain knowledge about diabetes, healthy eating, benefits
of physical activity, diabetes medications, and home blood glucose
monitoring.
Three classes with information about methods to manage diabetes.
Patients should plan to attend all three classes.
Class 1: Tuesday, July
7 2:00-4:30 PM
Class 2: Tuesday, July
21 2:00-4:30 PM
Class 3: Tuesday, July
28 2:00-4:30 PM
